Forms The forms needed for requesting a records transfer can be obtained below. In either of the formats you can type directly into the computer. If you want to keep an electronic copy for your records, use the MSWord format. Print out the completed form, obtain the necessary signature, and send it to the University Records Archivist, 101 Conrad Hall. If you have any difficulty with the process or you are still not able to view or use a form properly, call the UAHC, 355-2330. If you would like to save an electronic version of your form you will need to use the WORD format.
