APPENDIX F

November 15, 1994

Dear OPHS student,

Have you noticed the changes in the Office of Programs for Handicapper Students (OPHS)? We are now located in 120 Bessey Hall. We have stressed more communication with our students and, hopefully, you have received the Harbinger each month.

We have been busy lately making sure that our office satisfies your needs. What do you think? Are we doing a good job? Have any suggestions? We would like to know. We also are interested in your satisfaction with your overall MSU experience as a handicapper student.

Enclosed you will find a satisfaction survey. Please take a few minutes to complete it. Questions #1 through #12 ask for responses with your experience with the OPHS. Questions #13 through #15 ask for responses with your experiences with your overall University experience. We ask that you return the survey in the enclosed self- addressed envelope by Friday December 2, 1994. If you live on campus, you may drop it in the interdepartmental mail. So that you can express what you feel, you are not asked to identify yourself. All of our materials are available on alternate formats upon request.

If we can be of any assistance with your educational program at Michigan State University, feel free to stop in or give us a call at 353-9642.
